Case Manager

Role purpose

Perioperative care

• Verify consent forms, confirm pre-anesthetic assessments, and educate patients about surgery and preparation.
• Assess medical history, medications, allergies, and psychosocial factors for patients scheduled for surgery.
• Discuss management plans with treating physicians and ensure bookings and diagnostic tests are completed promptly.
• Coordinate individualized perioperative plans with surgical, anesthesia, and nursing teams; manage referrals, extra payments, and device availability to help avoid cancellations.
• Maintain patient records and resolve operating-room delays through communication with relevant parties.

Inpatient care and discharge

• Assess patient needs on admission and through referrals and unit rounds; facilitate multidisciplinary care-plan meetings.
• Create and adjust individualized care plans based on ongoing assessments and patient feedback.
• Start discharge planning at admission and assess medical, functional, and social needs for post-discharge care.
• Coordinate rehabilitation, home health, outpatient follow-up, and other required services before discharge.
• Identify barriers such as transport, finances, or insufficient home support and help address them.
• Advocate for patients and connect patients and families with community resources, support services, and financial assistance.

Education and experience

• Bachelor of Science in Nursing or MBBS degree.
• At least two years of experience in the same field.

Licences and certifications

• Licence from the country of origin and practice licence from the applicable regional health regulator, such as SCFHS or DHA.
• BLS, ACLS, and other applicable LSTC courses or certifications.

Skills and language

• Computer literacy and case management software proficiency.
• Excellent organization, leadership, attention to detail, interpersonal communication, critical thinking, and case-management skills.
• Empathy, active listening, problem-solving, and time management.
• Excellent English is required; Arabic is a plus.
