Education and Licenses
- Bachelor degree in Social Science or an arts degree in Social Work with no experience required.
- SCHS registration or license is required.
Job Purpose
Provide social work services for patients and families in resolving problems arising from medical conditions and circumstances.
Key Accountabilities and Responsibilities
- Visit wards and units regularly and establish professional relationships with medical and nursing staff.
- Ensure treating physicians and nurses understand the patient's social circumstances.
- Interview patients, family members, and friends to assess social needs and background.
- Prepare and implement management plans to reduce adverse effects on patients and use available resources effectively.
- Educate patients and families about medical and social management plans.
- Train and share knowledge with staff and students.
- Refer patients to social welfare agencies after reviewing their cases.
- Help resolve treatment barriers by arranging financial, social, and psychological support.
- Refer eligible patients to the Ministry of Social Development for disability benefits, financial aid, and rehabilitation equipment.
- Prepare reports and statistics with feedback and remarks for the Social Work Supervisor.
- Provide home on-call support for emergency abuse, neglect, suicidal attempt, and social or family cases.
- Assess psychosocial needs and provide emotional support and counseling to patients and families coping with illness, trauma, or loss.

- Respect patients and their families and promote a patient-centered care culture.
- Participate in quality improvement and patient safety activities individually or as part of a multidisciplinary team.
- Perform other job-related duties as assigned.
