Patient Flow Coordinator

Role overview

Responsibilities

• Identify patients who are willing to participate in and would benefit from Providence Healthcare services and initiate admission across inpatient and outpatient programs.
• Facilitate appropriate repatriation to acute care organizations for patients discharged from Providence Healthcare services.
• Promote communication with referring organizations and identify, reduce, or remove barriers to access.
• Monitor and support relevant operational and strategic targets for patient flow and outcomes.

Qualifications

• Be an Ontario-regulated health professional in good standing; the required clinical knowledge, skill, and judgment are those of a registered occupational therapist or physiotherapist.
• More than five years of relevant clinical experience and extensive knowledge of rehabilitation, including care for frail older adults and complex and marginalized populations.
• Demonstrated project management and team leadership experience.
• Knowledge of RM and R, e-stroke application processes, community resources, healthcare legislation, utilization pressures, and commonly used clinical assessment tools.
• Local Health Integration Network familiarity is an asset.
• Excellent critical thinking, interpersonal, diplomatic, communication, and problem-resolution skills; able to make rapid decisions.
• Proficiency with databases, spreadsheets, and software; willingness to learn and commitment to continuing education.
• Very good attendance and good performance records.
