Requirements
- Education/Certifications
- High School Diploma or GED
- Wyoming State certification for Peer Specialist
- BLS Certification within 30 days of hire
- Valid driver’s license
- Lived experience with SUD and/or mental health conditions, to include documented diagnosis
- Minimum two (2) years in successful recovery
- Competent computer skills, including Microsoft Office
- Maintains stable recovery and necessary care/treatment of behavioral health challenges and is willing to self-identify publicly as being in recovery from a mental health, substance use, or addiction diagnosis.
- Obtains/maintains state certification for Peer Supporters.
- Understands the concept of recovery as it relates to behavioral health; peer support/recovery coaching; wellness and recovery planning.
- Facilitates patient engagement and follow-up care.
- Documents in-person, video, and telephone encounters in electronic health record and stays current on all necessary documentation.
- Facilitates referrals for clinically indicated services outside the primary care clinic (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care).
- Participates in multi-disciplinary, team-based care of patients/clients.
- Participates in training, reporting, and other requirements of various grant supported programs implemented by One Health and partners.
- Exemplifies a patient-centered care approach to each patient along each stage of the patient journey, while working as a key patient centered team member by communicating individual patient care needs to support the needs of patients served.
- Provides exceptional customer service and models the mission of One Health when interacting with internal and external stakeholders promoting a positive image for the health center.
- Performs other duties as assigned or required with or without prior notification.
