Case Manager

Job Purpose

Perioperative Coordination

• Verify surgical consent forms, confirm pre-anesthetic assessments, and educate patients about the surgical process through direct communication and SMS.
• Assess medical history, medications, allergies, and psychosocial factors for patients scheduled for surgery.
• Discuss management plans with treating physicians and ensure bookings and diagnostic tests are completed promptly.
• Coordinate with surgical teams, anesthesiologists, and nursing staff on individualized perioperative care plans.
• Manage referrals, extra payments, device availability, and operating-room delays to help avoid cancellations.

Admissions and Care Planning

• Evaluate patients upon admission using referrals and unit rounds to identify needs and determine care requirements.
• Facilitate multidisciplinary meetings to review patient plans, progress, and discharge needs.
• Create and adjust individualized care plans based on ongoing assessments and patient feedback.
• Maintain detailed records of patient interactions, care plans, outcomes, and regulatory compliance.

Discharge Planning and Advocacy

• Start discharge planning at admission by evaluating potential post-discharge needs and suitable care settings.
• Coordinate rehabilitation, home healthcare, outpatient follow-up, community resources, support services, and financial assistance.
• Identify and address transportation, financial, home-support, medical, functional, and social barriers to safe discharge.
• Advocate for patient concerns and preferences throughout the hospital stay and coordinate all required follow-up services.

Continuous Improvement and Compliance

• Participate in quality improvement initiatives for pre-operative processes and discharge planning.
• Follow safety protocols, maintain confidentiality, comply with cybersecurity policies, and uphold organizational conduct and ethical standards.
• Complete mandatory education before required credentials or training expire.

Qualifications

• Bachelor of Science in Nursing or MBBS degree.
• Minimum of two years of experience in the same field.
• License from the country of origin and practice authorization from the applicable regional health authority, such as SCFHS or DHA.
• BLS, ACLS, and other applicable life support training course credentials.

Skills and Languages

• Computer literacy, organization, leadership, attention to detail, critical thinking, and interpersonal skills are required.
• Excellent command of English is required; Arabic is a plus.
