Service Coordinator Supervisor - Long-Term Support Services

As a Supervisor on our Long Term Support and Services team, you’ll lead a dedicated unit that supports individuals in accessing services and long-term support solutions—most often referred to as waivers—through Pennsylvania’s Community HealthChoices program. You’ll guide a team of Service Coordinators as they help individuals navigate complex systems, coordinate care, and promote independence through person-centered planning. This role is ideal for someone who brings strong leadership, empathy, and an understanding of how to manage both people and processes in a high-accountability environment.

Core Responsibilities
- Ensure that service plans are developed and implemented
- Ensure monitoring and follow-up are conducted appropriately
- Verify waiver eligibility and Medical Assistance requirements
- Support employees in meeting documentation and billing standards
- Monitor unit productivity and provide coaching as needed
- Oversee the incident management and reporting process
- Ensure timely and accurate completion of assessments and planning documents
- Provide direct supervision and caseload support when necessary
- Complete required trainings and prepare program reports
- Maintain cooperative and professional relationships with colleagues and stakeholders
What You Bring
RN, or a Pennsylvania-licensed social worker or licensed mental health professional
At least three (3) years of relevant experience
Supervisory, leadership, or management experience preferred
Ability to manage time effectively and respond to changing priorities
A valid Pennsylvania driver’s license, automobile, and required clearances

Location & Schedule
- This position is based in the Berks County area, with remote work and community-based service. Office hours are Monday through Friday, 8:30 AM to 4:30 PM.

Requirements
- RN, or a Pennsylvania-licensed social worker or licensed mental health professional
