RN Case Manager (Part-Time or Full-Time | 32–40 hrs/week)

Your Role

• As a key member of our interdisciplinary care team, you will work under the supervision of a Clinical Manager to coordinate and deliver high-quality, patient-centered care. You’ll play a vital role in ensuring comfort, dignity, and quality of life for those we serve.
• Perform ongoing, comprehensive assessments of patients and families
• Develop and oversee individualized care plans in collaboration with the care team
• Lead and participate in interdisciplinary case conferences
• Coordinate care with physicians, hospice staff, volunteers, and community partners
• Advocate for patients and families, addressing clinical, emotional, and psychosocial needs

Qualifications

• Active Massachusetts RN license in good standing
• Experience providing compassionate care in home or facility-based settings
• Strong interpersonal and communication skills
• Ability to work independently while thriving in a team environment
• Sound clinical judgment, flexibility, and strong organizational skills
• Valid driver’s license and access to a reliable, insured vehicle

• The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. • Pay Range$47—$56 USD

Preferred Qualifications
