RN case manager hospice must have experience

Responsibilities

• Provide skilled nursing assessments, care planning, and case management for hospice patients in accordance with company policies and regulatory requirements.
• Coordinate with interdisciplinary team members, patients, and families to deliver individualized care plans.
• Ensure timely and accurate documentation of patient care and case management activities.
• Advocate for patients’ needs and preferences while promoting comfort and dignity.
• Maintain compliance with all regulatory standards and company protocols.
• Communicate effectively with patients, families, and healthcare team members to promote continuity and quality of care.

Requirements

• Valid RN license with current registration.
• Proven experience as a Case Manager in hospice care.
• Strong knowledge of hospice guidelines, regulations, and best practices.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
