Responsibilities

Administer medications and treatments as prescribed by physicians
Monitor patient health and record vital signs
Assist patients with daily living activities and personal hygiene
Collaborate with healthcare professionals to develop and implement care plans
Educate patients and families on health care and wellness
Maintain accurate and timely medical records
Ensure compliance with all healthcare policies, procedures, and safety standards

Requirements

Valid LPN license in the state of Michigan
Previous experience in home care or a similar nursing role is a plus
Strong knowledge of nursing practices and medical procedures
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Compassionate and patient-centered approach to care
Reliable transportation to travel to patient homes
