This will include working with data from across primary, secondary and care sectors, as well as data on wider determinants of health, to undertake segmentation modelling, risk stratification, analysis of full patient pathways, and identification and monitoring of health inequalities.

The postholder will work with stakeholders across health and care, to use linked data to tailor the provision of care to identify people who would benefit most from proactive intervention and to tailor services to best meet population need.
