Physician

Provide direct medical care and consultation to homebound primary care patients within established productivity guidelines

Monitor patient conditions and adjust treatment plans based on clinical needs and responsiveness

Serve as a compassionate, professional representative of the agency to patients, families, and community partners

Collaborate with colleagues across the interdisciplinary team

Contribute to both formal and informal staff education

Participate in community outreach efforts to promote Visiting P services

Maintain accurate, timely documentation in the Electronic Medical Record

Ensure compliance with NJ Department of Health regulations and all relevant licensing and accrediting requirements

Adhere to all infection control and safety policies

Uphold the organization’s mission through excellence, professionalism, and positive teamwork

Job Competencies & Minimum Qualifications :

Licensed as a physician in New Jersey with education and experience in primary care settings

Geriatric experience recommended

Board certified in family practice or internal medicine

Active CDS and DEA license

Excellent verbal and written communication skills

Excellent organizational and interpersonal skills

Car and valid NJ driver’s license

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $200,000.00 to USD $296,000.00.
