We are seeking a PRN Physical Therapist who is passionate about delivering exceptional, one-on-one patient care and contributing to a supportive, team-focused environment.

This role offers flexibility and the opportunity to support our Schaumburg clinic and surrounding Northwest suburban locations.

• Perform comprehensive initial evaluations in accordance with APTA standards
• Develop and implement individualized plans of care based on evaluation findings

• Monitor patient progress and communicate effectively with referring providers and care teams
• Educate patients through customized home exercise programs
• Manage discharge planning and ensure continuity of care
• Maintain compliance with payer and documentation standards
• Stay current with best practices through continuing education and professional development
• Collaborate with healthcare providers, case managers, and insurance partners
