  • About Staying HomeCare Staying HomeCare is a California-licensed Home Care Organization (HCO License #194701215) providing concierge-level private duty home care through our SHC Signature Care Program, including Elder Care, Ability Care, and Recovery and Transition Care. Our mission is to deliver exceptional, personalized care that allows clients to remain safely and comfortably at home, supported by families who trust us completely. We are a boutique, relationship-first agency. Every client and family receives our full attention. The Care Manager is the primary point of contact for clients and families on all matters related to care quality, and the direct supervisor of caregivers in the field. You will work closely with the Office Manager, who handles scheduling and administrative operations, and report directly to the agency owner. This role is ideal for a seasoned home care professional who leads with empathy, communicates confidently with families, and holds caregivers to a high standard of care delivery. What You Will Manage Client Intake and Care Planning
  • Conduct in-home assessments for all new clients to evaluate care needs, environment, safety, and family dynamics
  • Develop individualized care plans that reflect each client's preferences, routines, medical considerations, and goals
  • Match clients with caregivers based on skills, personality compatibility, language, and schedule
  • Guide families through the onboarding process and set clear expectations for care delivery
