Job Summary

Job Qualifications

• Master’s degree in Social Work (MSW) from an accredited school of social work
• Current Level One Fingerprint Clearance Card
• Registered as an Associate Clinical Social Worker with the Arizona Board of Behavioral Health Examiners
• Prior experience in a healthcare setting
• Familiarity with regulatory requirements related to:
• Demonstrated skill in patient assessment, care planning, intervention, and evaluation
• Strong communication skills (verbal, non-verbal, written, and oral)
• Excellent customer service and patient relations skills

Job-Specific Duties

• Conduct comprehensive psychosocial assessments of patients and families
• Develop, implement, and regularly update individualized care and discharge plans
• Provide counseling, crisis intervention, and supportive services to patients and families
• Collaborate with the interdisciplinary healthcare team to coordinate patient care
• Educate patients and families on available community resources and support services
• Make mandated reports and ensure compliance with all legal and regulatory requirements
• Maintain accurate, timely, and compliant clinical documentation
• Participate in team meetings, case conferences, and quality improvement activities
• Placement in the pay range is based on multiple factors, including but not limited to relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for CMC Phoenix roles, including but not limited to shift differential and other special pay practices. The posted compensation for the position is a reasonable estimate that extends from the lowest to the highest pay that CMC Phoenix in good faith believes it might pay for this particular job, based on the circumstances at the time of posting.
