- Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
- Manage pain and symptom control to maximize patient comfort and quality of life
- Coordinate with interdisciplinary team members including physicians, social workers, and chaplains
- Provide education and emotional support to patients and their families throughout the end-of-life process
- Document clinical findings, care plans, and patient progress in compliance with hospice regulations
- Respond to after-hours patient needs and on-call situations as required
- Collaborate with community resources and care facilities to ensure seamless patient transitions
- Active RN license in the state of Massachusetts
- Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
- Strong knowledge of pain management, symptom control, and end-of-life care principles
- Excellent communication and interpersonal skills with patients, families, and care teams
- Ability to work independently in a field-based, patient home environment
- Valid driver's license and reliable transportation required
- CPR certification and up-to-date clinical credentials
