- Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
- Provide skilled nursing visits in home, facility, and residential settings across the Braintree, MA area
- Manage pain and symptom control using evidence-based palliative care practices
- Educate and support patients, families, and caregivers on disease progression and end-of-life care
- Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides
- Document clinical findings, care plans, and patient updates in accordance with Medicare/Medicaid hospice guidelines
- Provide on-call nursing support and respond to urgent patient needs as needed
- Active RN license in the state of Massachusetts (required)
- Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
- Strong knowledge of pain management, symptom control, and end-of-life care protocols
- Excellent communication and interpersonal skills with patients, families, and care teams
- Ability to work independently, manage a caseload, and prioritize in a fast-paced environment
- Valid driver's license and reliable transportation for field-based visits
- Proficiency with electronic medical records (EMR) documentation
