Concierge Home Care is hiring a Home Health Physical Therapist (PT) . If you’re looking for more control over your schedule, meaningful patient relationships, and the ability to increase your income based on productivity—this is the role for you.

Coverage Area: W inter Garden, FL / Orange County, FL ( Territories designed to minimize drive time with clustered scheduling )

Compensation & Earning Potential

  • Pay-per-visit model: $70+ per visit + Medicare differential pay
  • Average full-time earnings: $95,000 – $120,000+ with a flexible schedule
  • Quarterly bonus opportunities
  • Mileage reimbursement + data plan stipend

Your Role as a Home Health Physical Therapist

  • Evaluate patients and develop individualized treatment plans
  • Provide therapeutic exercises and mobility training
  • Educate patients and caregivers on safety and recovery
  • Collaborate with physicians and interdisciplinary teams
  • Supervise Physical Therapist Assistants (PTAs)
  • Complete documentation using streamlined technology

Qualifications

  • Compact or Florida Physical Therapist license
  • Valid driver’s license, reliable transportation, and auto insurance
