Home Care Nurse

Job Purpose

Key Duties and Responsibilities

• Provide home nursing care according to physician-prescribed treatment and care plans.
• Assess patient health, monitor vital signs, and report clinical concerns to physicians.
• Perform nursing procedures within licensed scope and demonstrated competency.
• Administer prescribed medications and treatments according to safety policies.
• Collect ordered blood and clinical specimens within licensed scope.
• Verify patient identity before services and clinical procedures.
• Document home visits, assessments, procedures, treatments, and observations.
• Follow infection prevention, sterilization, and PPE procedures.
• Educate patients and families on conditions, medications, home care, and warning signs.
• Report emergencies and sudden deterioration under emergency protocols.
• Maintain equipment and safely dispose of medical waste and sharps.
• Coordinate with the healthcare team and participate in quality improvement activities.

Qualifications and Requirements

• Bachelor's Degree or Diploma in Nursing from a recognized institution.
• Valid DHA professional nursing license.
• Home healthcare or clinical nursing experience is preferred.
• Valid Basic Life Support certification.
• Knowledge of infection prevention, control procedures, and patient safety.
• Excellent communication and interpersonal skills.
• Ability to work independently with professional responsibility.
• Commitment to confidentiality and professional ethics.
