Community Care Supervisor - 14 Month Contract

Hours and contract

• Monday to Friday, 8:30 a.m. to 4:30 p.m.
• 14-month contract position with an as-soon-as-possible start date.

Service area and travel

• Position serves the north service area: Newmarket, Keswick, Sutton, and Pefferlaw.
• Frequent travel is required within the service area and to the Aurora head office.

Position summary

Responsibilities

• Communicate with clients, families, case managers, and staff regarding schedules, care-plan goals, and service delivery.
• Adhere to organizational policies and procedures.
• Participate in an on-call rotation.
• Monitor, train, and assist Personal Support Workers delivering personal support and homemaking services.
• Complete initial and ongoing assessments of new clients.
• Promote organizational services in the community.

Qualifications

• Bachelor's degree in Registered Nursing or Registered Practical Nurse qualification.
• Be registered and in good standing with the College of Nurses of Ontario.
• Two years of management experience in a multidisciplinary, culturally diverse home health care environment.
• Knowledge and experience with CCAC.
• Own transportation and a valid driver's licence.
• Ability to travel frequently within the service area and to the Newmarket office.

Pay

• Pay range is $62,000.00 to $68,500.00 per year.
