Position summary

The Client Care Supervisor manages the overall delivery and functioning of Personal and Home Support Services.

This is a 14-month contract position in the West service area, including Woodbridge, Kleinburg, and Schomberg.

Start date is as soon as possible.

Duties

Communicate with clients, families, case managers, and staff about scheduling, care-plan goals, and service delivery.
Follow the organization's policies and procedures.
Participate in the on-call rotation.
Monitor, train, and assist Personal Support Workers delivering personal support and homemaking services.
Complete initial and ongoing client assessments.
Promote the organization's services in the community.

Qualifications

Bachelor's degree in Registered Nursing or Registered Practical Nursing.
Current registration in good standing with the College of Nurses of Ontario.
Two years managing in a multidisciplinary, culturally diverse home health care environment.
Knowledge and experience with CCAC.
Own transportation, valid driver's licence, and ability to travel frequently within the service area and to the office.
Computer knowledge of MS Office, Excel, Word, and PowerPoint.
Detail orientation, independence, leadership, supervisory, relationship-building, customer-service, organizational, time-management, facilitation, negotiation, and problem-solving skills.

Schedule

Monday to Friday, 8:30 a.m. to 4:30 p.m.
Frequent travel is required within the service area and to the head office in Aurora.

Pay

Pay is $62,000 to $68,500 per year.

Work location

Work is on the road in the West service area; frequent travel to the head office in Aurora is required.
