We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills.

Your Role in our Company Will Be
- The LVN supports the Director of Nursing, Case Management and the nursing staff in the delivery of health care by performing patient care assessments, and a variety of medically related tasks. The LVN is a licensed nurse who works in an Home Care Setting to provide nursing services.
AVAILABILITY
Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer)
RESPONSIBILITIES
- Providing nurse visits to home health patients. (home visits)
- Care for ill, injured, or convalescing patients by Implementing plan of care initiated by RN
- Provide basic patient care and treatment. Document consistently with the plan of care.
- Communication with patients to schedule visits.
- Educating the patient how to take care during non-visit days
- You'll demonstrate knowledge of regulatory requirements.
