Social Worker

Exciting New Opportunity with Affirmed Home Care! Affirmed Home Care is seeking a compassionate and motivated Social Worker (MSW) to join our team in Nassau County.

This is a per diem position with the potential for a full-time opportunity as the program grows. The Social Worker will provide rotating onsite social work coverage across three hotel-based client service locations in Syosset, Plainview, and Woodbury.

The Social Worker will play an important role in supporting clients with benefits navigation, including entitlements, housing coordination, care coordination, behavioral health support, discharge and hospital coordination, and connections to community resources. The Social Worker will also assist clients with accessing food assistance programs, transportation services, and other resources that promote long-term stability and independence.

Master’s Degree in Social Work (MSW)

1+ year of experience in healthcare or social services; home care and substance use experience preferred

Strong case management and care coordination skills

Excellent communication and interpersonal skills

Ability to manage multiple clients and priorities in a fast-paced environment

Compassionate, patient-focused approach to care

Ability to work independently while collaborating effectively with a multidisciplinary team
