What you'll Do

• As an Admissions Nurse, you will play a vital role as both a clinical expert and a trusted guide for patients, families, and healthcare partners:
• Serve as the primary nurse responsible for evaluating patients referred for hospice services
• Educate patients and families on hospice care and clearly communicate all components of our continuum of care
• Collaborate with referring physicians and liaisons to coordinate individualized, hospice‐appropriate plans of care
• Act as a key liaison between Trustbridge Hospice and hospitals, physicians, long‐term care facilities, and other healthcare professionals
• Promote and advocate for the hospice philosophy through education and relationship‐building within the healthcare community
• Your work ensures patients receive timely, compassionate care while helping families feel informed, supported, and confident in their hospice journey.
• What You'll Need
• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• Possess the ability to assess physical and psychosocial needs of patient and family.
• Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
