Registered Nurse (RN) - PRN

We’re looking for a compassionate and skilled Registered Nurse (RN) who thrives in a more personal, patient-focused setting.

In this role, you’ll have the autonomy to manage your own patients, build meaningful relationships, and deliver high-quality care in the home—whether supporting recovery or providing comfort-focused hospice care.

• Provide skilled nursing care, assessments, and treatments in patients’ homes
• Develop and manage individualized care plans for home health and/or hospice patients
• Monitor patient progress and adjust care as needed
• Collaborate with physicians, caregivers, and the interdisciplinary team
• Educate patients and families on care, medications, and disease progression
• Complete timely and accurate clinical documentation

• Active RN license (state of practice)
• 1+ year of clinical experience (home health, med/surg, or critical care preferred)
• Strong clinical judgment and communication skills
• Ability to work independently and stay organized
• Valid driver’s license and reliable transportation
