Overview
This is a full-time, on-site role,
providing direct nursing care to patients in their homes.
Day-to-day responsibilities include assessing patient conditions, monitoring vital signs, administering medications and treatments as prescribed, and performing wound care and other clinical procedures.
The nurse will develop and update individualized care plans, document all observations and interventions accurately, and communicate regularly with physicians, multidisciplinary teams, patients, and family members.
Additional duties include educating patients and caregivers on disease management, medication adherence, and home safety, as well as responding promptly to changes in patient status.
The role requires consistent adherence to clinical protocols, infection control standards, and regulatory requirements, while maintaining a respectful, patient-centered approach.
