Registered Dietitian - RD

Conduct comprehensive nutrition assessments for residents, including evaluation of medical conditions, dietary needs, weight trends, laboratory values, and nutritional risk factors.

Develop, implement, and monitor individualized nutrition care plans in collaboration with physicians, nursing staff, residents, families, and interdisciplinary care teams.

Manage therapeutic diets, food preferences, texture modifications, and nutrition interventions to support resident health, safety, and quality of life.

Monitor residents for changes in nutritional status, including weight loss/gain, hydration concerns, malnutrition risk, and response to dietary interventions; update care plans as needed.

Provide nutrition education and guidance to residents, families, and facility staff while ensuring compliance with regulatory standards and long-term care nutrition requirements.
