JOB RESPONSIBILITIES:
- Clinical Oversight, Quality Monitoring & Management-Driven Responsibilities
- as assigned.
- Conduct audits and validate clinical documentation to ensure accuracy, consistency, regulatory compliance, and alignment with evidence-based nursing practice.
- Perform joint supervisory field visits with clinical field staff, observe skills competencies, and provide oversight and supervision to paraprofessional staff.
- Collaborate with partner organizations, care coordinators, and Interdisciplinary team to meet patient needs.
- Adhere to all clinical protocols, regulatory standards, and infection control guidelines.
- Supervise, instruct, and guide CHNs, Personal Care Aides, or Home Health Aides.
- Provide clinical leadership and operational oversight for assigned cases by coordinating care activities, monitoring service delivery.
QUALIFICATIONS:
- Graduate from an accredited School of Nursing. Associate's Degree in Nursing, required. Bachelor's Degree in Nursing, preferred.
- Current active and unrestricted license as a Registered Nurse in New York State.
- Completed NYSED-approved infection control coursework.
- Experience: Minimum of three (3+) years of clinical nursing experience, including 1-2 years in home care or a similar healthcare setting, preferred.
- Strong clinical assessment and critical thinking abilities, excellent communication skills and the capacity to manage multiple cases effectively.
- Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the
- PACE organization on working with a frail or elderly population upon hiring.
- Proficiency with computers and electronic health record (EHR) systems.
- Experience with long-term care or managed care systems beneficial.
- Vehicle and current New York State driver's license required.
- Be medically cleared for communicable diseases and have all immunizations up to date
