Physical Therapist

Evaluate patients and develop individualized treatment plans

Provide physical therapy in patients’ homes

Help patients improve mobility, strength, balance, and daily function

Monitor progress and adjust treatment plans as needed

Coordinate care with PTAs and other healthcare professionals

Complete accurate and timely documentation

Active, unrestricted Texas PT license

At least 1 year of PT experience

Current BLS/CPR certification

Valid driver’s license and reliable transportation

Strong communication and organizational skills

Comfortable working independently

Own device and reliable internet connection required to access your schedule and complete charting
