Personal Care Assistant

Personal Care Aide (PCA) Staff is responsible for assisting clients with their personal care needs, activities of daily living, home environmental support, and safety, including oversight and supervision for all VNS Health Personal Care clients in accordance with their Scope of Practice as a certified PCA.

They work under the supervision of the registered nurse.

• Assists clients with personal care activities, including bathing, mouth care, skin care, grooming, toileting, transferring, and ambulation.
• Assists clients with housekeeping tasks, including planning and preparing meals, feeding, light housekeeping, changing linens, making beds, and laundering clothes and bedding.
• Assists client with safety needs by using equipment (side rails, foot stools, wheelchairs, etc.).
• Assist clients with transportation to appointments, errands, and social activities.
• Reminds client to take their medications at prescribed times.
• Follows the Plan of Care as indicated.
• Provides emotional support and companionship to clients, engaging them in conversation and activities.
• Establishes and maintains effective communication regarding the client’s care with the client, family, and supervising healthcare professionals.
• Maintains accurate records of care provided, including activities, observations, and incidents.
• Completes all VNS Health Personal Care’s records and reports according to agency policy and procedure.
• Accurately documents services provided by VNS Health Personal Care policy on the day services are rendered.
• Communicates and works cooperatively with other members of the healthcare team.
• Successfully completed a New York State Health Department-approved Personal Care Aide training program.
• Prefer a minimum of six (6) months experience as a Personal Care Aide or Home Health Aide.
• Must be at least 18 years old.
• Must be able to lift 25 pounds; sit, stand, bend, stoop, pull, reach, lift, and perform repetitive movements of the upper extremities.

• One (1) year work experience preferred.

$19.86- $20.00 per hour
