Outreach Worker

You'll spend much of your time out in communities, building relationships and connecting people with the support they need.

Your role will include:

Identifying and engaging people who are rough sleeping or at risk of homelessness

Delivering assertive outreach and supporting people who may be reluctant to engage with services

Supporting people to access accommodation, healthcare, treatment and recovery services

Working closely with drug and alcohol services to reduce harm and improve wellbeing

Building strong partnerships with housing providers, local authorities and health services

Supporting individuals to identify strengths, build resilience and achieve personal goals

Advocating for people experiencing multiple disadvantage and ensuring their voice is heard

Maintaining accurate records and contributing to service performance and outcomes
