Number of Hours per Rotation: 14 shifts of 8 hours per 3 weeks

Salary or Pay Band: Pay Band Nurse Practitioner $52.590 to $62.910

Job Description: The Heart Function Clinic (HFC) Nurse Practitioner RN (NP) is an advanced practice nurse with knowledge and skills gained through additional education and experience.

The Nurse Practitioner blends an in-depth knowledge of nursing with the legal authority and autonomy to order diagnostic tests, prescribe pharmaceuticals and performs specific procedures in the management and treatment of heart failure patients in the HFC.

This positon will provide advanced nursing support and continuity of care for patients with heart failure.

The RN (NP) practices within a framework of evidence-based acute care, holism, health promotion, and disease and injury prevention.

The practice involves building partnerships with individuals, families, and communities.

This includes: supporting inter-professional collaboration; and participating in continuous improvements through evaluative processes/projects.

This role will allow more patients referred to the HFC to be seen in a timely manner, and receive the care they need delivered in an evidence-based, holistic manner.

• Bachelor of Science in Nursing (BScN)
• Master of Nursing (MN)

• Valid Class 5 driver's license

• Basic - Critical thinking skills
• Basic - Conflict resolution skills

Knowledge and Abilities

• Knowledge in community development and facilitation strategies
• Knowledge in program development and implementation
