  • Manage a caseload of MAPs participants
  • Collaborate with individuals to develop a Virtual Community Resource Map (VCRM), identifying key people, places, and activities
  • Train participants and their support networks on enabling technologies (e.g., smartphones, tablets, and applications) to support daily tasks such as cooking, banking, health management, and scheduling
  • Coach participants in independent living skills, job readiness, transportation, healthy living, and social engagement
  • Ensure all required documentation is completed accurately, thoroughly, and in a timely manner
  • Attend conferences, seminars, and required trainings as assigned
  • Cooperate fully with licensing authorities and departmental staff during inspections, inquiries, or investigations
  • High school diploma or equivalent
  • Minimum of two (2) years of full-time experience working in the IDD field or a related human services field
  • Strong ability to communicate effectively and professionally with individuals at all levels, both internally and externally
  • Ability to establish and maintain supportive, respectful relationships with individuals served
  • Strong proficiency in the use of technology and digital tools
  • Ability to present ideas and factual data in a clear concise manner
  • Ability to establish a comfortable and supportive relationship with individuals
  • Ability to read, write, and communicate effectively in English
  • Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
