LPN / LVN

We’re looking for a compassionate and dependable LVN/LPN who enjoys building meaningful relationships and providing hands-on care. In this role, you’ll support patients in their homes—helping them recover, manage conditions, or receive comfort-focused hospice care.

Provide direct patient care including treatments, monitoring, and support

Assist with care plans for home health and/or hospice patients

Observe and report changes in patient condition to the RN/physician

Educate patients and families on basic care and daily needs

Collaborate with the care team to ensure quality and continuity of care

Complete accurate and timely documentation

One-on-one patient care in a home setting

Active LVN/LPN license (state of practice)

1+ year of clinical experience (home health, hospice, or bedside preferred)

Strong attention to detail and communication skills

Ability to work independently and follow direction from RN/clinical team

Valid driver’s license and reliable transportation

Reliable and self-motivated
