  • Conduct comprehensive psychosocial assessments of patients and families.
  • Develop and implement individualized plans of care addressing psychosocial, emotional, and environmental needs.
  • Provide counseling, emotional support, crisis intervention, and bereavement support to patients and families.
  • Assist patients and families with community resources, financial assistance programs, advance directives, and healthcare decision-making.
  • Collaborate with physicians, nurses, chaplains, aides, and other interdisciplinary team members to ensure coordinated care.
  • Participate in care planning meetings and maintain accurate clinical documentation.
  • Advocate for patient rights and ensure compliance with hospice regulations and standards.
  • Assist with discharge planning and transitions of care when appropriate.
  • Maintain confidentiality and professionalism in all patient interactions.
  • Master's Degree in Social Work (MSW) from an accredited School of Social Work.
  • Current and active Licensed Social Worker (LSW) license in good standing.
  • Minimum of two (2) years of supervised social work experience in a healthcare setting working directly with individuals.
  • Knowledge of psychosocial principles, community resources, and healthcare systems.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Previous hospice, palliative care, home health, or long-term care experience.
  • Knowledge of end-of-life care practices and bereavement support services.
