This role is vital to the medical success of those we serve, offering a unique blend of hands-on clinical care and medical oversight . You will act as a medical guide during crises and a consistent advocate for individuals during critical care appointments .
  • In this role, you aren’t just a nurse; you are a teacher and a coordinator who builds lasting relationships with medical providers and families to ensure well-managed, proactive healthcare is received.
  • What YOU'LL Do:
  • Medical Monitoring: Monitor daily documentation for medical needs, identify health trends promptly, and perform health monitoring of documentation to ensure high scores in quality audits .
  • Care Coordination: Coordinate with pharmacies and doctors, complete medical appointments, and provide advocacy at critical care consultations .
  • Clinical Support: Provide hands-on care when needed, including wound care, ostomy care, catheter management, and monitoring critical care medical needs.
  • Education & Instruction: Teach medication administration refresher courses, lead clinicals for CMAs and Medication Managers, and provide on-site instruction for staff regarding care plans and medical protocols .
  • Administration: Update and maintain care plans and medical protocols while utilizing technology (Therap) for accurate documentation and regulatory compliance.
  • SCHEDULE: 7p-7a (36hrs/week), every other weekend

Requirements

  • What YOU’LL Need:
  • Education: BSN or ASN from an accredited nursing program .
  • Licensure: Active state nursing license in good standing, valid driver's license in good standing required.
  • Experience: At least 1-2 year of general nursing experience, experience supporting individuals with intellectual or developmental disabilities is highly valued.
  • Certifications: Current CPR and First Aid (FA) certification .
  • Technical Skills: Proficiency with computers and technology is required for daily documentation and task management .
  • Physical Requirements: Ability to push, pull, and lift at least 50 lbs
  • Soft Skills: Strong critical thinking, time management, and the ability to teach individuals from diverse cultures and learning styles
