Licensed Practical Nurse (LPN)

Affirmed Home Care is seeking a compassionate, dependable, and skilled Licensed Practical Nurse (LPN) to join our team and provide high-quality, one-on-one care to a client in Jersey City, New Jersey .

Provide compassionate nursing care according to the client’s individualized care plan

Administer medications and prescribed treatments

Monitor the client’s condition and promptly communicate any changes

Provide wound, catheter, and other skilled nursing care within your scope of practice

Maintain accurate and timely clinical documentation

Promote the client’s comfort, safety, dignity, and independence

Communicate professionally with the client, family members, and care team

Active New Jersey LPN License

1+ year of nursing experience preferred

Current CPR/BLS certification

Strong clinical, communication, and organizational skills

Reliable, compassionate, and professional

Comfortable working independently in a home care setting

Valid driver’s license and reliable transportation preferred

EHR/documentation experience preferred
