What You'll Do

  • As an Intake Coordinator, you will play a vital role in ensuring patients are seamlessly connected to the care they need.
  • Responsibilities include:
  • Managing incoming patient referrals
  • Coordinating communication with hospitals, physicians, facilities, and referral partners
  • Maintaining accurate referral records and documentation
  • Scheduling admissions and coordinating logistics
  • Collaborating closely with clinical and administrative teams
  • Providing exceptional customer service to patients, families, and healthcare partners
  • Supporting efficient and timely admissions processes
  • What We're Looking For:
  • Experience in hospice, home health, healthcare intake, or Medicare-related services
  • Administrative or office support experience
  • Strong working knowledge of Microsoft Office applications
  • Excellent communication and customer service skills
  • Strong organizational abilities and attention to detail
  • Ability to multitask and prioritize effectively in a fast-paced environment
  • A positive attitude and team-first mindset
  • Location
  • Candidates should reside in or near the Calhoun, Georgia area.

Qualifications

  • Experience in hospice, home health, healthcare intake, or Medicare-related services
  • Administrative or office support experience
