- Passport ( Photo Copy, If available)
- COVID ID Vaccination Record Note: If the Applicants has the Hepatitis B Vaccine (Need to Provide). Otherwise, the applicant needs to fill out the form and decline
- TB Spot Test/Chest X-Ray (If you have a Chest X Ray, it must be valid for 5 yrs
- Physical ( Health Examination from your Primary Doctor/ Physician ) Note: If you have none, we can provide you with an Employee Health Examination Form, that you could request your primary Doctor/Physician to fill out and sign.
