Discharge Planner
Role Overview
Discharge planning is the process of transitioning a patient from one level of care to the next.
Ideally, discharge plans are individualized instructions provided to the patient as they move from the hospital to home or instructions provided to subsequent healthcare providers.
Discharge planner reports off to the Social Work team and staff.

Minimum Qualifications
- Education: Bachelor's Degree preferred or 3 years of experience; Masters preferred
- Licenses: None Listed
- Certifications: Handle With Care within 30 days of Hire
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
