Community Support Specialist
Full Job Posting
There will be challenges
Just like families, disabilities come in different types. You will see many of them along with the mental health challenges in the Individuals (aka the Program participants) that we serve. You will also learn about the different agencies and resources we work with to help you provide support directly, or indirectly, to the Individuals.
There will be chores
You will support the needs of the Individuals (both minors and adults) by coordinating services, e.g. you will initiate and complete intake assessments which includes interviewing prospective new Individuals and their family members to determine needed services, organize schedules and appointments, and you will proactively maintain open lines of communications with all key stakeholders (e.g., the Individuals, parents, conservators, agencies, employers, etc.) as you complete daily case notes, quarterly progress reports and share recommendations to the Individual’s Service Plan based on your personal insights.
You will advocate for the Individual in court proceedings as part of the Judicial Intervention and Diversion Compliance Program. You will also assist in building community partnerships to enhance available resources for the Program in order to improve access for the Individuals and their families to all necessary community, physical, and mental health services.
There will be chats, C.A.R.E., & cheers
You will participate in all required training, team, and company meetings to build your knowledge, skills, and abilities to grow in the family. We like to promote from within our own house!
You will take ownership of your own behaviors, and help coach others over time, in how to effectively demonstrate the company’s C.A.R.E. values (compassion, adaptability, respect, and empowerment).
You will receive ongoing coaching and performance feedback and be recognized for your personal and team contributions in addition to demonstrating the C.A.R.E. values.
