Assist participants in navigating health care, social services, and community support systems.
Provide education on health, wellness, prevention, and healthy lifestyles.
Coordinate referrals and follow up to help ensure participants receive needed services.
Document participant interactions and maintain accurate records.
Participate in outreach events and activities that promote health and community engagement, and collaborate with hospital departments and community organizations.
Maintain confidentiality and comply with all hospital policies and procedures.
