Community Hab - Direct Support Professional

This position is less than 20 hours /week

Pay rate $18.45/hr

• High school diploma or general education degree (GED).
• A valid NYS driver's license and ability to drive and driving record acceptable for agency insurance coverage are required.
• Access to a vehicle needed to provide services.

Who are we looking for: We are hiring people who enjoy working with others and will promote independence, self-esteem and community integration, to individuals with developmental disabilities.

Summary: As a community Habilitation DSP you are matched to an individual based on likes, location and availability.

Generally you will be going out into the community with the individual.

This position requires a great amount of commitment, self-motivation and solid communication skills.

Essential Duties and Responsibilities include the following (Other duties may be assigned):

The work is usually one to one

• Participates in a person centered approach to program plan development and participates in activities to build competencies and integrate individuals into their communities.
• Assists individuals we serve to identify social, recreation and other community resources the individual may wish to access and devise plans to achieve that access.
• Negotiates schedules and daily activities directly with the individual and/or caregiver. Establish and maintain regular hours with individual(s) that you work with
• Completes all mandated reporting requirements by Wildwood and regulating agencies.
