- Patient Assessment:
- Evaluate patients’ mental, emotional, and social needs, including family dynamics, financial situation, and support systems.
- Care Planning:
- Develop and implement care or discharge plans in coordination with doctors, nurses, and other healthcare staff.
• Counseling & Emotional Support
- Provide guidance to patients and families dealing with stress, trauma, chronic illness, or end-of-life situations.
- Discharge Coordination:
- Arrange post-hospital care such as rehabilitation, home healthcare, or placement in long-term care facilities.
- Resource Connection:
- Link patients with community resources, financial aid programs, housing services, or support groups.
- Advocacy:
- Ensure patients’ rights are respected and help them navigate healthcare systems and insurance processes.
- Crisis Intervention:
- Assist in emergency situations like abuse cases, mental health crises, or sudden illness.
