Case Manager - (CDCA Preferred)

On Demand Counseling is seeking a compassionate, organized, and motivated Case Manager to join our Newton Falls team.

This position plays a vital role in helping individuals with substance use and mental health disorders overcome barriers to treatment and recovery by connecting them with the resources and support they need to succeed.

• Assist clients with obtaining and maintaining basic needs, including food assistance, Medicaid, housing, identification documents, transportation, and other community resources.
• Coordinate referrals to medical, behavioral health, vocational, and social service providers.
• Collaborate with counselors and the multidisciplinary treatment team to support client progress.
• Assist with discharge planning and linkage to ongoing community supports.
• Advocate for clients and help them navigate complex systems and services.
• Maintain timely, accurate documentation in the electronic health record.
• Monitor client progress toward case management goals and follow up on identified needs.
• Develop positive working relationships with community agencies and referral partners.

• Must be at least 18 years of age.
• Successful completion of BCI/FBI background check and pre-employment drug screen.
• Minimum of a high school diploma or equivalent
• CDCA preferred (or willingness to obtain). We are happy to help the right candidate become credentialed.

• Experience working with individuals affected by substance use disorders, mental health disorders, or other vulnerable populations is preferred.
• Strong communication, organization, and problem-solving skills.
• Ability to work independently while collaborating effectively within a multidisciplinary team.
• Valid driver's license and reliable transportation preferred.
