Experience:
- Two years of experience in the ambulatory healthcare setting.
- Ideal candidate will have prior experience in population health initiatives such as chronic disease management, care management, or utilization management
- Skills and Abilities:
- Excellent communication skills (oral and written)
- Excellent customer service skills
- Proven leadership skills
- Ability to work independently, setting priorities to coordinate care plan efficiently
- Ability to work effectively in a team environment
- Highly organized and detail-oriented
- Efficient with MS Office – Outlook, Word, Excel, Teams
- Effective behavioral and educational strategies, including, but not limited to, motivational interviewing, teach-back method and self-management support
