The OhioRISE Care Coordinator is responsible for cultivating flexible, family-focused, community-based responsive services based on the High-Fidelity Wraparound model of care coordination.

Maintain fidelity to models of care coordination when conducting Intensive and/or Moderate Care Coordination activities.

Develop and maintain the Child and Family Team, including coordinating and leading team meetings.

Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update the plan according to Ohio Department of Medicaid and agency requirements.

in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related sector human services or behavioral health care field, providing community-based services to children and youth, and their family or caregivers.

High School Diploma plus 3 years’ experience in at least one of the above.

Associate or bachelor’s degree plus 2 years’ experience in at least one of the above.

Master’s degree plus 1 year experience in at least one of the above.

Valid Driver License, Auto Insurance and the provision of a vehicle for work purposes.
