Requirements

• The ideal candidate will possess the following qualifications:
• High school diploma or GED AND AT LEAST 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science required.
• 2+ years’ experience in public or private human service, with 1 year being direct care required. Experience must be with clients over the age of 18.
• Valid Driver's license required.
• All clearances required by OMHSAS regulations.
• Preferred qualifications:
• Bachelor's degree in sociology, social welfare, psychology, gerontology, or other human services.
• Under the direction of the Blended Case Management Supervisor, you will:
• Serve as the primary contact for up to 30 individuals in the community
• Develop, coordinate, and implement personalized service plans
• Provide case management, service planning, and coordination with other agency providers
• Offer supportive counseling to help individuals achieve their goals
• Work alongside county reentry grants to support individuals transitioning from jail back into the community
• Participate in on-call responsibilities as needed
