Education

• Postsecondary (Cert/Diploma/Program Grad) from an accredited school of nursing (Required) Or
• Associate’s Degree in nursing (Required)

Licenses/Certifications

• LPN - Licensed Practical Nurse - State Licensure in the state of practice Upon Hire (Required)

Experience

• 1+ years in related healthcare field (Required) And
• Prior Experience in case management or discharge planning (Preferred)
